Getting paid to care for a parent in West Virginia

The short answer: West Virginia's door is Personal Options — the self-directed option inside the WV Aged and Disabled Waiver (ADW); also available in the TBI and IDD waivers. Palco is the fiscal agent that puts the family caregiver on payroll., run by WV Department of Human Services, Bureau for Medical Services (BMS), with the WV Bureau of Senior Services operating ADW; Acentra Health processes medical-necessity applications; Palco is the financial management services vendor (since April 1, 2024). Pay: Medicaid pays $6.75 per 15-minute unit for ADW personal attendant care — $27.00 an hour. That rate went up from $6.36 ($25.44/hr) on 10/1/2025, and BMS's current ADW rate chart, effective 4/1/2026, still shows $6.75, so it is unchanged as of August 2026. Important: $27.00/hr is the BILLING rate — what Medicaid pays — not what you take home. In Personal Options your parent sets your hourly wage themselves, inside a monthly budget built from their assessed needs, and the state's rule is that your wage cannot exceed the Medicaid reimbursement rate. In practice it lands well below $27/hr, because that same budget also has to cover employer payroll taxes and other costs. West Virginia publishes no official average take-home wage; secondary sources put typical caregiver pay around $13/hr (uncorroborated — treat as a ballpark, not a promise).

Who can be the paid caregiver

Under Personal Options, the member (your parent) is the employer and may hire adult children, siblings, other relatives, friends, or neighbors as paid personal attendants. Hired staff must pass a background check and meet ADW training requirements. Excluded: the member's spouse, legal guardian, or the legal representative who makes decisions on the member's behalf — so if you hold guardianship or medical power of attorney acting as decision-maker, you may be blocked from being the paid worker. In the state-plan Personal Care program (agency-only, no self-direction), a family member can be paid only if a licensed home care agency hires them; spouses are prohibited there too.

The spouse question

No. WV Medicaid does not pay spouses: the official ADW site states plainly that staff hired under Personal Options "cannot be a spouse or legal guardian," and the Personal Care program also bars legally responsible persons. A spouse caring for a veteran is the exception — VA PCAFC and Veteran-Directed Care can pay spouses. For an adult child caring for a parent (Resbit's core question), this rule usually doesn't bite.

Waitlist reality

Short by national standards, but ADW is not an entitlement — the CMS-approved waiver caps enrollment at 8,750 people a year (plus 76 slots held back for people moving out of a nursing home), and when it's full a "Managed Enrollment List" forms, ordered by the date the medical-necessity (MNER) form was completed. The state's own site reports the average ADW waitlist time last fiscal year was 44 days, with the full application-to-services process averaging about 56 days. The state-plan Personal Care program is an entitlement with no waitlist ever — but treat it as a narrow backstop, not an easy fallback. It's agency-managed only (family can be paid there only if a licensed home care agency hires them), it requires help with at least 3 activities of daily living, and its income limit is much tighter than the waiver's: about $994/month for a single person in 2026, versus about $2,982/month for ADW. Most people who qualify financially for ADW will be over the income limit for Personal Care.

Worth knowing: Disqualifiers to state plainly: (1) spouses and legal guardians can never be the paid worker in any WV Medicaid program — including the adult child IF they are the parent's court-appointed guardian or acting legal representative; (2) the parent must medically qualify at nursing-home level of care (5+ functional deficits on the Pre-Admission Screening) — needing only light help usually fails; (3) Medicaid financial limits apply (~$2,982/mo income, $2,000 assets single, 2026); (4) an ADW member can't also work as a paid caregiver in another waiver. The $27.00/hr figure is the Medicaid billing cap, not the caregiver's paycheck — in Personal Options the family sets the wage within a monthly budget that also covers employer taxes, so actual hourly pay is meaningfully lower (secondary sources suggest low-to-mid teens; no official figure exists — this one detail is estimate-grade). Recent changes: HCBS rates rose ~6% effective 10/1/2025 under Gov. Morrisey; Palco replaced the prior fiscal agent on 4/1/2024, so older articles naming PPL/Public Partnerships are outdated. WV's ADW waitlist (~44 days average) is genuinely short compared to multi-year waits in many states — a selling point worth stating. Note the givers.com claim that WV waivers pay spouses is wrong per the state's own site; do not repeat it.

How to apply — step by step

  1. 1. Have your parent's doctor (or you, with the doctor's sign-off) complete the Medical Necessity Evaluation Request (MNER) form from adwprogram.wv.gov and send it to Acentra Health-ADW, 1007 Bullitt Street, Suite 200, Charleston, WV 25301, fax 866-212-5053.   2. 2. Acentra Health schedules an in-home Pre-Admission Screening; your parent must show at least 5 functional deficits (help needed with bathing, dressing, mobility, etc.) to qualify medically. Acentra's own line is 1-844-723-7811.
  3. 3. Apply for Medicaid financial eligibility at the county DoHS office or online at wvpath.wv.gov (2026 limits: about $2,982/month income, $2,000 assets for a single applicant; general Medicaid line 877-716-1212).
  4. 4. Once approved for ADW, tell the case manager you want the Personal Options (self-directed) model instead of a traditional agency.
  5. 5. A Resource Consultant helps build the spending plan; Palco (palcofirst.com/programs/west-virginia) registers your parent as an employer and enrolls you as the paid worker — background check and training required.

Start logging your shifts in the electronic visit verification (EVV) system Palco and your Resource Consultant set you up with — ask Palco which one, because West Virginia doesn't publish the vendor name anywhere we could find. Palco runs payroll and withholds taxes; you never handle the Medicaid money directly.
