Getting paid to care for a parent in Iowa

The short answer: Iowa's door is Consumer Choices Option (CCO) under Iowa's HCBS waivers — for an aging parent, the HCBS Elderly Waiver + CCO. (Iowa's old route, individual Consumer-Directed Attendant Care/ICDAC, ended December 31, 2025; CCO is now the self-directed path.), run by Iowa Department of Health and Human Services (Iowa HHS) / Iowa Medicaid, delivered through Iowa Health Link managed-care plans; payroll handled by Veridian Fiscal Solutions. Pay: Under CCO the member is the employer and sets the caregiver's hourly wage from an individual monthly budget (payroll taxes and fees come out of that budget, so take-home is lower than the gross wage). The state's official benchmark for families moving from ICDAC to CCO is $4.03 per 15-minute unit = $16.12/hour (code T1019), posted by Iowa HHS June 2025 for the Jan 1, 2026 transition. Agency-employed attendant care pay is whatever the home care agency offers. No statewide fixed wage exists for CCO employees generally.

Who can be the paid caregiver

Only the member's spouse (and a parent/stepparent of a member under 18) is barred from being the paid caregiver. Guardians and legal representatives CAN be paid caregivers, with limits (e.g., 40 hrs/week cap and rate cap for legal representatives under 441-78.37). The guardian/POA/representative-payee bar applies to the Independent Support Broker role, not the caregiver.

The spouse question

No. Iowa Administrative Code (441-77.33 and parallel waiver rules) requires the paid person to be "not the spouse of the member" — this applies to both attendant care and CCO self-directed personal care. An adult child caring for a parent is fine; a husband or wife caring for their own spouse cannot be paid through Iowa Medicaid.

Per the Dec 2022 Iowa HHS report: five waivers had waiting lists ranging from ~960 (Children's Mental Health, oldest 2022) to ~6,862 (Health & Disability, oldest 2018); Brain Injury 2,483 (oldest 2017), Intellectual Disability 5,514 (oldest 2018), Physical Disability 2,161 (oldest 2021). Elderly Waiver: no waiting list (still true per Jan 2026 sources). Figures are from a 2022 report; check current status given the ongoing HOME redesign.

Worth knowing: Big recent change: Iowa killed the individual CDAC provider model (the way most families got paid for 20 years). No new ICDAC providers after July 1, 2025; all disenrolled December 31, 2025 (IRS-compliance reasons). Any guide telling families to 'enroll as an individual CDAC provider' is now wrong — the path is CCO (or getting hired by an agency). Disqualifiers: spouses can never be the paid caregiver under Iowa Medicaid; guardians/POAs/representative payees are barred; the parent must need nursing-facility level of care and meet financial limits (2026: ~$2,982/mo income, $2,000 assets single; home largely exempt up to $752,000 equity). CCO caregivers are the member's employees — the member (or family) carries employer duties (timesheets, budget discipline), with Veridian Fiscal Solutions doing payroll and an Independent Support Broker helping (transition members meeting training/timeliness requirements can skip the ISB). One secondary site (medicaidplanningassistance.org) claims spouses can be hired under CCO — the Iowa Administrative Code says otherwise; we follow the code. Iowa's HOME Project waiver redesign is ongoing (CareStar assessments began 2025), so program mechanics may shift again; confirm details at 800-338-8366 when applying.

How to apply — step by step

  1. 1. Apply for Medicaid + the HCBS Elderly Waiver: online at the Iowa HHS Services Portal (hhsservices.iowa.gov), at your local HHS office, or call Iowa Medicaid Member Services at 800-338-8366. For help navigating, Iowa Compass (the state's ADRC) is 800-779-2001.
  2. 2. Complete financial eligibility (2026: about $2,982/month income and $2,000 in countable assets for a single applicant; spousal protections apply) and a level-of-care assessment — since 2025 done by CareStar, Iowa's independent assessor. Your parent must need nursing-facility level of care. Approval can take up to 3 months.
  3. Level-of-care assessments are done by Iowa's Core Standardized Assessor (CSA) contractor — CareStar, Inc. as of July 1, 2026 (succeeding Telligen, whose contract ended June 30, 2026), using interRAI tools.
  4. 4. The case manager reviews CCO responsibilities and refers you to Veridian Fiscal Solutions; you receive employer and employee packets. Work with an Independent Support Broker (or the case manager) to build the monthly budget.
  5. 5. Hire your family member as the employee: complete the packet, pass a background check (one done within the past 24 months counts), set the hourly wage within the budget. Veridian runs payroll and pays the caregiver from the Medicaid budget.
